Here are some additional tips courtesy of Andrew Trout, MD:
1. Push harder (need to displace gas)
2. You are not done until you have done a sweep of the deep pelvis to look for fluid, inflammation, collections
3. Change the bladder if you cant find the appendix - if it's full, empty it, if it's empty, let it fill
4. The bladder can be used for counter pressure - if the caecum or appendix are down in the pelvis, pressure on the left side of the bladder with your left hand can lift them out of the pelvis into the RLQ
5. For the retrocecal appendix, creep out to the right flank or position left lat decubitus

This is the hardest ultrasound to be done I'd rather do other jobs the appendix isn't meant for purpose and scanning I swear to God
