Well, I think that the 4th ending would be if the player doesn’t drink the pills. I think the pills are what caused him to hallucinate

Maybe the secret ending was, in the hero ending, to NOT take the pills from the doc. The instructions at the very begining says endings can be define by our actions or INACTIONS.

Dr. Pleasant talking about the DDDR made me realize he's based off of/inspired by Samuel Loomis and Michael Myers. That was the whole thing with Loomis and Myers. Michael was "pure evil," and Dr. Loomis treated him when he was admitted. Plus, he looks almost just like Dr. Pleasant. I'm a huge Michael Myers fan, so I kinda fangirled when I realized this

I love the inspiration this game has from Halloween. As soon as I saw Dr.Pleasant and heard him talking about how the patient escaped and was evil and will kill again; I instantly thought of Dr.Loomis from Halloween
