The Sophie's Choice bit was a joke on both the movie as well as the expression itself. The expression basically is about difficult choice because both options are about to same - either both outcomes roughly equally good, or more typically and usually, because both of them are really really bad. The movie was based on a novel of the same name and Meryl Streep won an Oscar for her role in it playing a Polish Jewish woman who's forced to choose between two of her children in an Nazi concentration camp as to who will live and who will die. In other words, she's basically deciding which of her two children is being put to death. Gervais' joke of course being that since they're making a sequel, and since she already chose one child to be put to death in the original movie (the younger one, the girl), in the sequel it won't be much of a choice now since it's just one child left, so I guess he will be it. It's a dark joke - typical of Ricky.
